Abstract

|
목적: 본 연구는 간호간병통합서비스의 간호인력 배치의 적정성을 평가하기 위함이다.
방법: 간호간병통합서비스 데이터를 국민건강보험공단으로부터 제공받아 이를 분석하였다. 데이터베이스에서 환자의 간호요구도와 관련있는 병원데이터와 환자 데이터를 추출하였고, 이를 의료기관 종별, 간호인력배치 수준별 간호요구도의 차이를 분석하였다.
결과: 약 70%의 의료기관은 표준배치수준, 즉, 상급종합병원은 1:6, 종합병원 1:10, 병원 1:12로 간호서비스를 제공하였다. 환자의 특성 또한 병원 유형에 따라 유의한 차이를 보였으며, 주진단의 경우 상급종합병원과 병원에서 완전히 다른 형태로 나타났다. 상급종합병원은 중증도와 일상생활의존도로 평가한 간호요구도가 높은 배치수준에서 가장 높게 나타났다. 그러나 종합병원과 병원의 경우 간호요구도와 간호배치수준은 관련성이 적게 나타났다.
결론: 질높은 간호서비스를 제공하기 위해서는 정확한 업무량 예측이 선행되어야 하며, 정확하게 예측한 업무량을 기반으로 간호배치수준이 제시되어야 한다.

Purpose: This study was done to evaluate the adequacy of nurse staffing in integrated nursing care.
Methods: Statistical data on integrated nursing care from the National Health Insurance Corporation was used in this study. We extracted hospital data and patient data related to patient needs for nursing care. We analyzed the differences in patient needs by staffing level of each type of medical institution.
Results: Approximately 70% of medical institutions provided nursing care by the mid level of nurse staffing, which was 1:6, 1:10 and 1:12 in the tertiary hospital, general hospital and semi-hospital, respectively. The patients’ characteristics were significantly different by hospital type. Especially, the distribution of the main diagnosis was completely different between the tertiary hospital and semi-hospital. In the tertiary hospital, the patient needs measured by severity and activities of daily living dependency were higher at higher staffing level than at lower staffing level. However, the nurse staffing was less relevant to the patient needs in the general hospital and semi-hospital.
Conclusion: To provide high-quality nursing care, accurate workload forecasting is required at the start, and then the standard nurse staffing level can established based on workload forecasting.
